Person-Centered Care Model

Personalized Care to Nurture The Spirit
Behind every resident is a spirited passion for something. At Pleasant Valley, our caregivers nurture that passion by fostering a trusted relationship through person-centered care.
Each of our dementia trainers has been carefully selected and is a graduate of our company\’s Train the Trainer program facilitated by a certified dementia consultant. Continually learning new techniques in caregiving for those living with dementia, our trainers provide caregivers with the tools needed to establish heart to heart connections through person-centered care.

PERSON-CENTERED CARE EMPOWERS
OUR RESIDENTS

When seniors transition from their home to a nursing home, the loss of independence and being out of their comfort zone can take a toll, which is why our philosophy encourages choice, purpose, and meaning in residents’ lives – helping them to feel valued. Our caregivers work hard to build a rapport with residents though encouragement, praise, conversation, and by getting to know each person – their personality, children, careers, likes, and dislikes.

We focus on building upon the strengths and abilities residents have and encourage them to be as independent as possible – from putting on a shoe to outdoor nature walks or playing piano. No matter the level of cognition or physical ability, we provide meaningful activities, love, and cognitive stimulation that help our residents be the best version of themselves. With Pleasant Valley’s person-centered care, residents will live an improved quality of life filled with self-respect and personal connections.
